Ages: 7-12 years old

To register, fill out the form, and there are 4 options available right now:
Payments can be cash, or check payable to "SLO Blues Baseball"
1.) Can drop off form and payment in person (Cash, Check or Credit Card) at our office, Monday - Friday during business hours. We are located at 241 Prado Rd in San Luis Obispo (Silverado Stages). Click here for Driving Directions to SLO Blues Office.
2.) Can mail in registration form and payment to (should probably do this option before June 16th to allow time for mail service):

3.) Can fax in completed registration form to (805) 543-2667,or drop off form at office, to reserve spot, and then submit payment during check-in the 1st day of camp (Wednesday, July 19th, 8:30am - 9:00am)
4.) Submit registration form and payment during check-in the 1st day of camp (Wednesday, July 19th, 8:30am - 9:00am)
It is recommended to complete and submit form as early as possible so we can forecast headcount for camp and to ensure we have the correct t-shirt size for the camper.
